My health for life Program availability in Western Queensland
My health for life is available across Western Queensland through structured telephone health coaching and is also available in small group sessions in several locations.
Telephone health coaching
Telephone coaching is a flexible option, allowing you to participate in the program at a time and location that works for you.
You will have six appointments with your health coach over six months. Appointments one to five are held fortnightly, with a 12-week break between sessions five and six where you will put into practice what you've learnt so far.
How do you join the telephone coaching program? To find out if the program is the right fit for you, you will need to complete the health check. You can do this online, myhealthforlife.com.au/risk-assessment or over the phone with a My health for life team member at 13 74 75. If you qualify for the program, the team will be in contact to enrol you in the program.
Group program
Group programs are delivered by local health professionals who have been trained as My health for life Facilitators. They offer a positive and supportive environment to share and learn as a group, while working on your own personal health goals.
You will have six sessions with your group over six months. The first session is a one-on-one appointment with your health coach (either face to face or by phone) where your coach will gain an understanding of your needs and goals. Sessions run fortnightly and you will have a 12-week break between sessions five and six.
