Mental Health Nurse initiative
The Mental Health Nursing in General Practice (MHNiGP) will play a major part in enhancing local services to support care for people with severe and complex mental illness who are managed in the Western Queensland practice population. This project will locate “clinical care coordinators” within practices to coordinate mental and physical care needs as well as connect people to community services. The MHNiGP will support better access to specialist support services for GP’s and ACCHO’s and will improve assessment and referral pathways from acute, secondary and social care sectors.
