Heart Health Checks: The Critical role of the practice nurse

Health Professional Webinar

The Heart Foundation are hosting a webinar in collaboration with the Australian Primary health Care Nurses Association to focus on the critical role of practice nurses in implementing Health Health Checks.

Approximately 76,500 cardiovascular disease related events could be prevented with the uptake of Heart Health Checks over the next five years.

The new Heart Health Check MBS item provides general practitioners and primary health care nurses with an incentivised opportunity to initiate an early and continuing relationship with their at-risk patients not already known to have CVD.

General practice nurses are at the heart of care

Join the webinar for practical advice on:

  • implementation of the new Heart Health Check MBS item
  • the role of the practice nurse in supporting absolute CVD risk assessment
  • new primary care data outlining gaps in practice around CVD risk factor assessment
  • system level nurse operated strategies to maximise quality improvement through the primary prevention of CVD

Thursday 6th February 7:00 - 8:00pm AEDT
