Care Finders in WQPHN

Western Queensland PHN has released a tender to the market on 20 December 2022 for new Care Finders to become operational from February 2023 across the entire WQPHN catchment.
  • Care Finders will provide specialist and intensive assistance to help people within the Care Finder target population to understand and access aged care and connect with other relevant supports in the community. Care Finders will complement, and should not duplicate, My Aged Care. Care Finders will specifically target senior Australians who need intensive support to understand and access aged care as well as connect with other relevant supports in the community. This will include people who are not yet receiving aged care services, as well as those who are.

People who are eligible for aged care services and have one or more reasons for requiring intensive support to:

  • Interact with My Aged Care and access aged care services
  • Access other relevant supports in the community

WQPHN Care Finder Market Briefing

The Australian Government is funding Primary Health Networks across Australia to commission the new care finder program. A single Care Finder Market Briefing will be delivered by WQPHN online, Wednesday 11 January 2023 at 10am AEST.

The market briefing is your opportunity to learn more about the program, the commissioning and contracting process, to build your understanding of PHNs probity obligations and learn about the next steps following the tender closing period on 23 January 2023.
