2021 WQPHN Health Needs Assessment Survey

WQPHN is conducting a Health Needs Assessment (HNA) to better understand service gaps, key issues and regional priorities. The information will be used to inform local commissioning of services and healthcare improvements for Western Queensland.
The survey can be completed by both community members and service providers/clinicians/stakeholders and will ask slightly different questions depending on your selection.
KBC Australia have been engaged to work with WQPHN to develop the 2021 HNA.
The survey should take approximately 3 minutes to complete.
Survey and Confidentiality
The survey is completely confidential and data will not be linked back to individual survey responses.
Your input is highly valued. The resulting HNA document will be published on www.wqphn.com.au in early 2022.
