Client Need:
People at risk or living with addiction issues specifically Alcohol and Other Drugs.
Ages – all ages
Service Details:
- Alcohol and other drug intervention;
- Alcohol and other drug recovery services;
- Early intervention and treatment services;
- Brief and psychosocial interventions;
- Client centred approach;
- Withdrawal management support;
- After Care support;
- Coping strategies and self-care of significant others;
- Respite services-day program and residential rehab.
(Life on Track) / (Life Back).
Counselling and day-based group programs for young people
(Life on Track) and adults (Life Back).
How to access:
The following referrals are accepted under this program:- GP Referral
- Self-referral
- Family/carer referral
- Allied Health professional referral
- Govt and Non Govt AOD services;
- Community organisation referral
- Local Aboriginal Medical Services
- Criminal Justice System;
- Linkages with Local Hospital Health Services and acute Mental Health teams
