Central West QLD – Service Provider Directory

The Central West Queensland (CWQ) Service Provider Directory is a funded project of the Integrated Care and Commissioning (ICC) initiative, an Australian Government program jointly delivered by the Department of Health, Disability and Ageing, the National Disability Insurance Agency, and the Department of Veterans’ Affairs, with support from the National Indigenous Australians Agency, as well as state, territory, and local governments. This directory was developed in direct response to feedback from community members across CWQ, who expressed difficulty in identifying which services visit their area. The CWQ Service Provider Directory aims to provide a clear, locally relevant resource that highlights in-person and outreach services that are operating in Central West Queensland, making it easier for residents, carers, and service providers to find and connect with the support they need.

Integrated Care and Commissioning Initiative (ICC)

Western Queensland PHN is proud to be a trial site for the CWQ ICC initiative that brings together resources across health, aged care, disability, and veterans’ care to better support and coordinate services in rural, remote, and First Nations communities within the following Central West regions:

  • Barcaldine
  • Barcoo
  • Blackall Tambo
  • Boulia
  • Diamantina
  • Longreach and
  • Winton

This initiative supports communities to:

  • identify what better access to care looks like for them
  • design sustainable solutions
  • tailor care and support activities to local community needs.

WQPHN is currently one of 6 Integrated Care and Commissioning trial sites nationally.

This initiative is part of the Federal Governments reforms to strengthen Medicare.
