Western Queensland PHN is seeking an ambitious and tech-savvy health professional to lead digital innovation in primary care and improve heart health outcomes across regional communities.

About the Role

The Primary Care Digital Health Coordinator will lead the rollout of the PHASES project (Preventing Heart Attacks and Stroke Events) and champion the implementation of Primary Sense, a digital platform for quality improvement in general practice.


You’ll collaborate with primary care teams, internal stakeholders, and community partners to empower clinicians, improve patient outcomes, and embed digital solutions into everyday care. This role combines project leadership, digital innovation, and community impact in one dynamic position.


Ideally, this role will be based within the Western Queensland region, such as Longreach, Mount Isa, Winton or Roma; however, applicants are welcome to apply from across Queensland.

Key Responsibilities

  • Lead and champion the PHASES cardiovascular health project and Primary Sense adoption across Western Queensland.
  • Empower primary care teams with training, support, and change management for digital health initiatives.
  • Collaborate with internal teams to integrate digital health tools across chronic disease, aged care, and mental health programs.
  • Monitor and report on project outcomes, data quality, and meaningful use of health insights.
  • Build and maintain relationships with primary care providers and community partners to deliver sustainable, integrated care.


About You

We’re looking for someone who is proactive, collaborative, and driven to make an impact. You might be a great fit if you:

  • Have tertiary qualifications or experience in health, primary care, or health management.
  • Bring experience or interest in digital health platforms, clinical data, or quality improvement.
  • Understand the health needs of rural and remote communities, including Aboriginal and Torres Strait Islander Peoples.
  • Enjoy coordinating projects, empowering others, and driving change in healthcare.
  • Are organised, confident managing multiple priorities, and hold a current driver’s licence for regional travel.
